Silent Shadows in Endodontics
Rethinking Asymptomatic Persistent

Endodontic Lesions

This session will explore the findings from a recent prospective cohort study on patients with Asymptomatic Persistent Endodontic Lesions (APEL), conducted at the National University Centre for Oral Health, Singapore.

​

Part 1 evaluates the impact of a video-based decision aid on patients' treatment choices, decisional conflict and knowledge levels. In this study, a majority of patients presenting with APEL chose long-term monitoring of their tooth over endodontic intervention. Decision Factors influencing patients’ decision-making process were also studied. 

​

Part 2 compares 2D periapical radiographs and 3D CBCT imaging in the assessment of teeth with APEL, evaluating lesion size, proximity to anatomical structures, and presence of missed canals. Overall, CBCT provided a more comprehensive examination of teeth with APEL than periapical radiographs. A radiographic protocol for these asymptomatic lesions was developed.

Orofacial Pain
More than Toothaches 
- But What?

A common misconception is that all pain is a direct symptom of disease or tissue injury and can be cured by treating the underlying cause. While this model often holds true for acute orofacial pain (OFP) conditions including odontogenic types of pain, it is insufficient when applied to chronic OFP. Over recent decades, advances in pain research—from nociceptive signalling to brain imaging—have highlighted the complex, multidimensional nature of chronic pain and its intimate connection to general physical and mental health. Chronic OFP is best understood and managed within a biopsychosocial framework, yet translating this into individualized care remains a clinical challenge. This lecture will provide an overview on classification of chronic OFP conditions and explore the novel concept of keystone pain mechanisms, illustrating how mechanistic pain profiling may inform both assessment and tailored management strategies – a step towards personalized precision medicine.

DIGITAL ENDODONTICS - Relevant for Current Clinical Practice?

Digital technologies are increasingly shaping modern endodontic practice, offering enhanced diagnostic capabilities, procedural precision, and new opportunities for clinical efficiency. While tactile skill remains foundational, advancements such as cone beam computed tomography (CBCT) have redefined diagnostic imaging with regards to periapical pathosis and complex canal anatomies. Dental Magnetic resonance tomography (dMRI) has the potential for a relevant next step in diagnostics.

Other emerging tools include AI-driven decision-making enhancement, 3D printed guides for minimally invasive access, and dynamic navigation systems that integrate CBCT data into real-time visualisation during procedures. These innovations—though currently not fully implemented even specialist settings—signal a shift toward more predictable and streamlined treatment planning and treatment.

Digital enhancements in visualisation, such as microscope integration with heads-up displays, offer a futuristic glimpse of endodontics where data from multiple sources converge chairside. Additionally, digital peripherals such as connected motors, new anaesthesia delivery systems and potential digital pulp diagnostic tools aim to further improve patient care and clinical accuracy.

While many of these technologies remain in early adoption stages, their rapid development and integration potential highlight the relevance of digital endodontics today. Clinicians must stay informed and engaged with these trends to discern developments that may advance clinical practice and optimize outcomes in a digitally evolving landscape.
